Job summary

Affinity Hospice Management LLC is seeking a skilled Registered Nurse to plan, organize, and direct hospice care in home and community settings. The RN will build on community resources to meet patient and family needs, delivering care with emphasis on education and self-care support.

The role requires comprehensive assessments, nursing care planning, and coordination with a multidisciplinary team. On-call duties and ongoing professional development are part of the responsibilities.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed Registered Nurse in good standing.
• Experience in public health, home care, or hospice nursing preferred (1–2 years).
• Excellent observation, communication, and problem-solving skills.

Responsibilities

• Complete initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Write and initiate plan of care; regularly re-evaluate patient and family needs.
• Initiate and revise the plan of care as patient status changes.
• Use health data to determine nursing diagnoses and develop goals.
• Administer medications and treatments as prescribed.
• Counsel patient and family in nursing and related needs and provide health instructions.
• Assist with activities of daily living and support self-sufficiency.
• Coordinate patient care as Case Manager when assigned by supervisor.
• Complete accurate clinical notes and communicate with physicians and team.
• Participate in on-call duties and ensure equipment/services availability.
• Supervise ancillary staff and pursue ongoing professional development.
