Job summary

Affinity Hospice Management LLC in Biddeford, Maine seeks a registered nurse to plan, organize and direct home-based hospice care, bringing community health education and patient-centered services to individuals and families at home.

The RN will perform comprehensive assessments, develop and implement care plans, coordinate with physicians and the interdisciplinary team, provide caregiver education, and ensure safety and quality in all aspects of hospice nursing.

Qualifications

• Graduate of an accredited school of nursing with an active RN license.
• Minimum 1 year of professional nursing experience within the last 3 years; public health/home care/hospice preferred.
• Must have CPR certification and strong observation, communication and problem-solving skills.

Responsibilities

• Perform initial and ongoing comprehensive patient and family assessments for hospice needs.
• Provide professional nursing care using the nursing process and plan of care.
• Develop, revise and implement care plans with patient/family involvement.
• Coordinate with physicians and interdisciplinary team; ensure safe care environment.
• Educate patients and families on care needs, medications, and self-care techniques.
• Participate in on-call duties and maintain documentation of care provided.
• Drive to patient homes and manage equipment and supplies as needed.
• Adhere to regulatory requirements and maintain professional growth through education.
