Job summary

Heart n Home Hospice, part of LHC Group, seeks an RN Case Manager to deliver and evaluate the Plan of Care for hospice patients in home settings. You will collaborate with families, physicians, and the IDG to coordinate care and ensure quality outcomes.

Responsibilities include initial eligibility assessment within 48 hours, ongoing re-assessment, documentation of care, and adherence to hospice policies. Requires RN licensure and a valid driver's license; CPR and 1+ year clinical experience

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license and access to dependable transportation.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral.
• Assesses physical, psychosocial, and environmental needs and re-assesses every 15 days.
• Initiates and coordinates the plan of care with accurate clinical notes.
• Documents problems, goals, interventions and family responses.
• Collaborates with patient/family, attending physician and IDG daily.
