Job summary

Elite Hospice, part of LHC Group within the UnitedHealth Group family, is seeking a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients. You will perform evaluations, collaborate with clinicians, and implement treatment plans to promote physical, psychosocial, emotional, and spiritual well-being.

The role requires an active RN license, a valid driver’s license or access to reliable transportation, and CPR certification (or ability to obtain within 90 days).

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation
• 1+ years of clinical experience
• Current CPR certification or ability to complete within 90 days of hire

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral.
• Identifies patient/family needs and reassesses every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and responses.
• Collaborates with patient/family, attending physician and IDG daily.
• Instructs and supervises patient/family in self-care when appropriate.
• Maintains accurate clinical notes.
• Provides care irrespective of age, race, creed, color, sex, disability, or financial condition.
