Job summary

Elite Hospice, part of LHC Group and a member of the Optum family, seeks an RN Case Manager to oversee hospice Plan of Care delivery and ensure quality of care for patients and families. You will collaborate with professional staff to implement treatment strategies and maintain thorough documentation in a home setting.

Responsibilities include initial evaluations, care planning, ongoing assessments, and patient/family education.

Qualifications

• Current and unrestricted RN licensure in the state of practice (Arkansas).
• Current driver's license with reliable transportation.
• Preferred: 1+ year of clinical hospice/home health experience.
• Preferred: CPR certification or ability to obtain within 90 days.

Responsibilities

• Make the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify physical, psychosocial, and environmental needs and reassess every 15 days.
• Initiate and coordinate the plan of care in collaboration with the IDG.
• Document problems, goals, interventions, and patient/family responses.
• Educate and supervise patient/family in self-care and maintain accurate clinical notes.
• Collaborate with family, physician and IDG to deliver daily patient care.
