Job summary

Heart n Home Hospice, part of LHC Group within the Optum family, is hiring an RN Case Manager to oversee the plan of care and ensure quality hospice services in the community. You will perform initial evaluations, coordinate care, document progress, and collaborate with the IDG.

Requirements include RN licensure in the state, a valid driver's license or reliable transportation, CPR preferred or obtainable within 90 days, and 1+ years of clinical experience.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license and access to reliable transportation.
• CPR certification preferred or can be obtained within 90 days.
• 1+ years of clinical experience.

Responsibilities

• Perform initial nursing evaluation within 48 hours of referral.
• Identify needs and reassess at least every 15 days.
• Coordinate the plan of care and document progress.
• Collaborate with patient/family, physician, and IDG.
