Job summary

Elite Hospice, part of LHC Group and a member of the UnitedHealth Group family, seeks a Registered Nurse Case Manager to oversee hospice patients’ plans of care and collaborate with the IDG to promote physical, psychosocial, emotional, and spiritual well-being.

You will perform initial evaluations, coordinate care, document progress, and educate patients and families on self-care within diverse home settings. A valid RN license and driver’s license are required.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver’s license with valid vehicle insurance and access to a dependable vehicle or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.
• 1+ years of clinical experience preferred.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral to determine eligibility for hospice services.
• Identifies physical, psychosocial, and environmental needs and reassesses every 15 days.
• Initiates and coordinates the plan of care; documents problems, goals, interventions, and responses.
• Collaborates with patient/family, attending physician and IDG to provide daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes about the patient’s condition.
