Job summary

Heartland Hospice is seeking a Hospice RN / RN Case Manager to lead in-home patient care. You will assess, plan, and deliver skilled nursing services, coordinating with physicians, LPNs, CNAs, social workers, and chaplains to ensure compassionate, comprehensive care in patients' homes or other settings.

Required is an RN license in the employment state and CPR, with hospice or home health experience preferred. A valid driver's license and auto insurance are also required.

Qualifications

• Current RN license in the state of employment.
• Hospice or home health experience preferred (1 year minimum).
• CPR certification required; valid driver’s license and auto insurance.

Responsibilities

• Be the primary contact for patients and families, guiding them through hospice care.
• Deliver and document skilled nursing care at home, in assisted living, or patient’s residence.
• Perform initial and ongoing assessments to monitor condition and adjust care as needed.
• Collaborate with an interdisciplinary team to build personalized care plans.
• Provide emotional, physical, and spiritual support to patients and families.
