Job summary

Heart n Home Hospice, part of LHC Group and a member of the Optum family, is seeking a Registered Nurse Case Manager to deliver the Plan of Care for hospice patients during home visits and coordinate care with the IDG. This role emphasizes compassionate, patient-centered care in diverse home environments.

The ideal candidate holds an unrestricted RN license in the state, a valid driver's license, and reliable transportation; CPR certification is preferred within 90 days of hire.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to dependable transportation.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies the patient/family needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care with accurate clinical notes.
• Documents problems, goals, interventions, and patient/family responses to hospice care.
• Collaborates with patient/family, attending physician and IDG for daily care.
