Job summary

Heart to Heart Hospice is seeking an RN Case Manager to lead comprehensive patient assessments and coordinate care for individuals with life-limiting illnesses. You will work with patients and families, collaborate with physicians and IDG teams, and ensure timely documentation and care plan updates.

The role emphasizes on-call coverage and strict adherence to healthcare regulations and company procedures. A nursing license and reliable transportation are essential.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License and reliable transportation (auto liability insurance).
• Two years of RN experience preferred in healthcare/clinical setting.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patients and determine the impact of terminal illness on physical, psychosocial, and environmental needs.
• Implement and revise the individualized plan of care (POC).
• Coordinate care among patient, family, physicians and healthcare professionals.
• Attend IDG meetings and participate in care planning and prioritization of needs.
• Provide on-call service and timely documentation submission.
• Comply with local, state, and federal laws, Medicare regulations, and internal policies.
