Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize and direct hospice care in home and community settings. The RN conducts comprehensive patient and family assessments, develops plans of care and coordinates with the interdisciplinary team to deliver compassionate, goal-oriented care.

The role emphasizes case management, education for patients and families, on-call responsibilities, and supervision of hospice aides as needed.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• RN with at least 1 year of professional nursing experience within the last 3 years, or BSN from NLN-accredited program with current RN license.
• Preference for 2+ years’ experience, including public health, home care or hospice nursing.

Responsibilities

• Completes initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Initiates and revises the plan of care as patient needs change.
• Coordinates patient care as Case Manager when assigned by Clinical Supervisor.
• Communicates with physician and care team; documents notes and reports changes.
• Teaches patient and family self-care; provides health instructions.
• Participates in on-call duties and quality improvement activities.
• Supervises ancillary personnel and ensures equipment and services are available.
