Job summary

Affinity Health Management seeks a Registered Nurse to plan, organize and direct hospice care in patients' homes and communities. The role emphasizes community health education and coordination of care across the care team.

The RN will perform comprehensive assessments, develop and revise care plans, administer medications, and educate patients and families about care options and self-care techniques within a home-based hospice setting.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• RN with at least 1 year of professional nursing experience within the last 3 years; or a BSN with RN license.
• Management experience not required; may supervise hospice aides.

Responsibilities

• Provide comprehensive assessments of patients and families to determine hospice needs.
• Deliver professional nursing care using the nursing process.
• Develop and revise plans of care with patient and family involvement.
• Administer medications and treatments as prescribed by the physician.
• Provide patient and family education and counseling on care needs.
• Participate in interdisciplinary team meetings and ensure continuity of care.
• Participate in on-call duties and coordinate with care team as needed.
