Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize, and direct hospice care within patients' homes and communities. The RN will assess patients, develop care plans, administer treatments, and educate families, working with an interdisciplinary team to ensure high-quality palliative care.

The ideal candidate is a licensed RN with experience in home health or hospice and strong communication skills, able to coordinate care and participate in on-call duties to support

Qualifications

• Graduate of an accredited school of nursing.
• Licensed Registered Nurse in good standing.
• Experience in public health, home care, or hospice nursing preferred.

Responsibilities

• Complete initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Provide professional nursing care and administer prescribed medications and treatments.
• Develop and revise care plans with patient, family, and interdisciplinary team; coordinate patient care.
• Communicate with physicians, families, and team members; document notes and changes.
• Participate in on-call duties and ensure equipment and services are arranged.
• Maintain safe environment and uphold quality assessment and improvement activities.

Education

Graduate of an accredited school of nursing

Baccalaureate degree in nursing
