Job summary

Affinity Hospice Management LLC seeks a registered nurse to plan, organize, and direct hospice care in home and community settings. The role emphasizes comprehensive patient assessments, care planning, and coordination with families and the interdisciplinary team.

The RN will use nursing processes to deliver care, revise plans as patient needs evolve, and serve as case manager when assigned. Driving patients and effective communication are essential in this role.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with a License in good standing to practice.
• Minimum of 1 year of professional nursing within last 3 years; or BSN with 2 years in public health/home care/hospice preferred.
• Licensed driver with insured vehicle.
• CPR certification (hands-on).
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills.

Responsibilities

• Completes an initial, comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care by utilizing all elements of nursing process.
• Develops and revises plan of care based on nursing diagnosis and patient status.
• Counsels patient and family; provides health care instructions.
• Acts as Case Manager when assigned; coordinates patient care for caseload.
• Participates in on-call duties and ensures equipment and services are available.
• Supervises ancillary personnel and pursues continuing education.
