Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize and direct hospice care in home settings. The RN will perform comprehensive patient assessments, develop and revise care plans, administer medications, educate patients and families, and act as Case Manager as needed.

The role requires current RN licensure, graduation from an NLN-accredited program, and relevant experience, with emphasis on community health, home care or hospice nursing.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• RN with 1 year of professional nursing in last 3 years or BSN with current RN license; 2 years in public health/home care/hospice preferred.
• Management experience not required.

Responsibilities

• Completes and maintains accurate clinical notes.
• Communicates with physician regarding patient needs and orders.
• Coordinates care with the interdisciplinary hospice team.
• Provides patient and family education on nursing care and self-care techniques.
• Assists patient with activities of daily living and comfort measures.
• Participates in on-call duties and ensures equipment and supplies are available.
