Qualifications

• Graduate of NLN-accredited school of nursing.
• Current RN licensure in state.
• 1 year of professional nursing experience in last 3 years, or BSN with current RN license; 2 years in public health, home health, or hospice preferred.
• Management experience not required; may supervise hospice aides.

Responsibilities

• Complete initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Initiate and revise plan of care as patient status changes.
• Administer medications and treatments as prescribed by the physician.
• Counsel patient and family; provide care instructions.
• Coordinate patient care as Case Manager when assigned.
• Attend interdisciplinary group meetings.
• Participate in on-call duties.
