Job summary

MissionHires is seeking a Patient Care Advocate to drive referral growth for hospice services within an assigned territory. You will build strong partnerships with physicians, facilities, discharge planners, and community agencies while educating referral sources on hospice programs.

You will develop and execute territory plans, conduct sales calls, and act as a liaison between referral partners and internal teams to ensure timely, high-quality care for patients and families.

Qualifications

• Bachelor's degree in marketing or related field or equivalent professional experience.
• 2+ years hospice sales or related-industry experience with a track record of direct-sales growth.
• Strong oral communication, presentation, relationship-building, and self-management skills.

Responsibilities

• Own development and referral growth of an assigned territory, including facilities, physicians, care-at-home providers, and community agencies.
• Build and maintain productive relationships with physicians, payors, facility staff, discharge planners, social workers, and community partners.
• Develop, implement, and evaluate quarterly and annual territory plans to achieve growth goals.
• Conduct effective sales calls and presentations with targeted referral sources to develop partnerships and promote hospice services.
• Maintain knowledge of hospice services and act as a liaison between referral partners and internal teams.
• Meet with patients and families to discuss home care services and expectations.
• Travel locally to community locations, clinics, hospitals, patient homes, and offices to promote hospice services.
• Submit timely documentation and use agency software to complete assignments.
