Job summary

Heartland Hospice is seeking a Hospice Care Consultant to lead strategic outreach with referral partners across hospitals, clinics, and home health settings. You will advocate for hospice services, build durable relationships, and drive growth while ensuring patients and families understand early hospice options.

Ideal candidates bring 2+ years in healthcare sales, strong relationships with healthcare providers, and excellent communication and CRM skills.

Qualifications

• Bachelor's degree or equivalent experience required.
• Minimum 2 years in healthcare sales, business development, or referral marketing.
• Strong existing relationships with physicians, hospitals, SNFs, or home health professionals.
• Prior hospice/palliative care/home health experience a plus.
• Excellent communication, presentation, and time-management skills.
• Proficiency in Microsoft Office Suite and CRM platforms.
• Sensitivity to needs of terminally ill patients and families.
• Proven track record of meeting or exceeding admissions targets.

Responsibilities

• Educate, inform, and inspire confidence in our services to ensure the right patients are referred at the right time.
• Identify new referral opportunities by calling on hospitals, home health agencies, SNFs, assisted living, clinics, and physician offices.
• Engage in meaningful conversations with healthcare professionals about patient needs and hospice options.
• Plan and execute strategic marketing initiatives to increase awareness and drive referral growth.
• Represent Heartland Hospice at community events, associations, and educational sessions to promote services.
• Serve as a resource and advocate for patients and families, ensuring understanding of hospice options and benefits of early referrals.
