Job summary

Heartland Hospice is seeking a Hospice Care Consultant to lead referral development and partnerships in the healthcare community. The role focuses on educating providers, identifying referral opportunities, and advocating for hospice care to improve patient quality of life.

You will work with hospitals, home health agencies, SNFs, and clinics to grow referrals, leveraging your knowledge of hospice services and strong communication skills.

Qualifications

• Minimum 2 years in healthcare sales, business development, or referral marketing.
• Strong existing relationships with physicians, hospitals, SNFs or home health professionals.
• Prior hospice, palliative care, or home health experience is a plus.
• Bachelor’s degree in business, marketing, or communications (or equivalent).

Responsibilities

• Educate, inform, and inspire confidence in our services to ensure correct referrals.
• Identify new referral opportunities by calling on hospitals, SNFs, home health agencies, clinics, and physician offices.
• Engage healthcare professionals in conversations about patient needs and hospice options.
• Plan and execute strategic marketing initiatives to increase awareness and drive referrals.
• Represent the organization at community events, associations, and in-services to promote services.
• Serve as a resource for patients and families, explaining hospice options and benefits of early referrals.
