Job summary

Heartland Hospice, part of Gentiva, seeks a Hospice Care Consultant to drive referral growth by building relationships with hospitals, SNFs, home health agencies, and physician offices. You will educate professionals on hospice options and advocate for early referrals.

The role emphasizes strategic marketing, community engagement, and compassionate patient-family support, with a focus on quality outcomes and collaboration across clinical and operations teams.

Qualifications

• Bachelor's degree in business, marketing, communications or equivalent experience.
• Minimum 2 years in healthcare sales, business development, or referral marketing.
• Strong relationships with physicians, hospitals, SNFs or home health professionals.
• Prior hospice, palliative care, or home health experience is a plus.
• Excellent communication, presentation, and time-management skills.
• Proficiency in Microsoft Office Suite and CRM platforms.
• Sensitive to needs of terminally ill patients and families.
• Proven track record of meeting or exceeding admissions and census targets.

Responsibilities

• Educate, inform, and inspire confidence in our services to ensure right patients are referred at the right time.
• Identify new referral opportunities by calling on hospitals, home health agencies, SNFs, assisted living, clinics, and physician offices.
• Engage in meaningful conversations with healthcare professionals about patient needs and hospice benefits.
• Plan and execute strategic marketing initiatives to increase awareness and drive referral growth.
• Represent our organization at community events, professional associations, and in-services.
• Serve as a resource for patients and families, explaining hospice options and benefits of early referrals.
