Qualifications

Responsibilities

• Develops overall referral growth for the assigned territory including facilities, physicians, care-at-home providers, and community agencies.
• Builds and maintains relationships with providers and referral sources through calls and presentations.
• Implements quarterly and annual territory plans to achieve growth goals.
• Communicates program information and new services to referral partners and care teams.
• Travel locally to promote hospice services and educate healthcare providers.
