MissionHires seeks a Patient Care Advocate to drive referral growth for hospice services in the Dallas area. You will cultivate relationships with physicians, facilities, discharge planners, payors, and community partners, educating them on hospice programs and home care options.

Responsibilities include developing territory plans, conducting sales calls, presenting to referral sources, and coordinating with internal teams to ensure timely care and compliant operations.

Competenze

• Bachelor's degree in marketing or related field or equivalent professional experience.
• At least 2 years of hospice sales or related-industry experience with a track record of direct-sales growth.
• Strong communication, presentation, and relationship-building skills.

• Own the development and referral growth of an assigned territory with facilities and physicians.
• Build and maintain relationships with physicians, payors, discharge planners, and community partners.
• Develop and evaluate quarterly/annual territory plans to hit growth goals.
• Conduct sales calls and presentations to develop partnerships and promote hospice services.
• Maintain knowledge of hospice services and liaison with internal teams on offerings.
• Meet patients and families to discuss home care services and needs.
• Partner with Intake and care resource teams to ensure timely communication with referral sources.
• Log daily call activity and document follow-up plans in agency systems.
• Participate in community education and marketing activities.
• Prepare monthly referral-source tracking reports and communicate market changes to leadership.
