Job summary

Gentiva Hospice is seeking a Hospice Care Consultant to lead strategic outreach with hospitals, home health agencies, and physician offices. You will educate referral partners, advocate for patients, and drive growth through relationship-based sales and targeted marketing initiatives.

The role emphasizes independent work and collaboration with clinical and operations teams to improve patient access to hospice services and enhance quality of life for patients and families.

Qualifications

• Bachelor's degree in business, marketing, communications or equivalent experience.
• Minimum 2 years in healthcare sales, business development, or referral marketing.
• Strong existing relationships with physicians, hospitals, skilled nursing facilities or home health professionals.
• Prior hospice, palliative care, or home health experience is a plus.
• Excellent communication, presentation, and time-management skills.
• Proficiency in Microsoft Office Suite and CRM platforms.

Responsibilities

• Educate and inspire confidence in services to ensure right patients are referred at the right time.
• Identify new referral opportunities by calling on hospitals, home health agencies, SNFs, clinics, and physician offices.
• Engage in meaningful conversations with healthcare professionals about patient needs and hospice options.
• Plan and execute strategic marketing initiatives to increase awareness and drive referrals.
• Represent Gentiva Hospice at community events and professional associations to promote services.
• Serve as a resource for patients and families, explaining hospice options and benefits of early referrals.
