Job summary

VitalCaring Group in Texas invites a Field-Based Care Transition Coordinator to connect patients, families, and healthcare partners, guiding seamless transitions into home health or hospice. This role emphasizes relationship-building, referral growth, and coordinated care across clinical teams.

You will travel within the assigned territory, engage referral sources, and work closely with hospitals, SNFs, and physician offices to improve patient access to services.

Qualifications

• Bachelor’s degree in Healthcare, Business, Marketing, or related field preferred (or equivalent experience)
• Minimum of two years of healthcare or related sales experience
• Strong communication, relationship-building, and influencing skills
• Ability to manage multiple priorities in a fast-paced, field-based environment
• Proficiency with Microsoft Office and CRM systems
• Demonstrated ability to work independently and drive results
• Valid driver’s license and reliable transportation

Responsibilities

• Serve as liaison between referral sources, patients, caregivers, and clinical teams to ensure seamless transitions
• Execute structured sales calls including pre-call planning and needs discovery
• Build and maintain relationships with referral partners through communication and service excellence
• Develop strategies to grow market share and drive referral volume
• Track and document sales activity across referral sources using CRM tools
• Achieve referral and admission goals
