Qualifications

• Current RN license in the state of employment.
• Active BLS for healthcare professionals from AHA or Red Cross.
• Minimum one year hospice or hospital nursing experience preferred.
• Experience using EMR/EHR systems.
• Mobile driver with valid license and auto insurance per policy.
• Reliable transportation to meet visit schedules.
• Ability to use equipment with visual/auditory mechanisms.
• Ability to communicate effectively in English (verbal and written).
• Flexible schedule to meet Hospice House patient needs.
• Ability to manage emotional stress with terminally ill patients and families.
• Ability to physically access patient rooms and navigate stairs/space; lifting as needed.

Responsibilities

• Provide direct care to patients and families as part of IDG.
• Coordinate the Plan of Care reflecting evolving patient needs.
• Educate patient/family about disease, palliative care, safety.
• Report changes in condition to IDG or other professionals.
• Evaluate hospice referrals/admissions for level of care.
• Attend patient deaths and prepare body for transport.
• Provide bereavement support with psychosocial staff.
• Present concise reports to IDG; encourage interdisciplinary input.
• Communicate with physicians, staff, patients, families, supervisors.
• Supervise nursing team and adapt during emergencies.
• Perform other duties and urgent nursing tasks as needed.
