Job summary

Chapters Health System, Inc. seeks a Per Diem RN to assess patient/family needs, coordinate the Plan of Care with the IDG, and provide palliative and supportive care. You will visit patients in Hospice House, hospitals, and home settings across assigned areas.

The role requires an active RN license in Florida, BLS certification, and ability to travel with reliable transportation. Strong communication in English and flexibility are essential for this demanding position.

Qualifications

• Current RN license in the state of employment.
• Active BLS from AHA or Red Cross.
• At least 1 year of hospice or hospital nursing experience preferred.
• Experience with EMR/EHR systems.
• Valid driver’s license and automobile insurance; reliable transportation.
• Ability to communicate in English (verbal and written).
• Flexible schedule to meet Hospice House patient needs.
• Ability to lift and meet essential physical requirements.
• PACE sites require Alzheimer's/Dementia training completion.

Responsibilities

• Provides and manages direct care to patients and families as part of IDG.
• Coordinates the Plan of Care to reflect evolving patient needs.
• Educates patient and family about disease, palliation, caregiving, safety.
• Reports changes in patient condition to IDG or physicians.
• Participates in evaluating hospice referrals/admissions for level of care.
• Attends patient deaths and prepares body for transport; oversees disposal of medications.
• Communicates with physicians, staff, patients, families, and supervisors.
• Supervises nursing staff when applicable.
• During emergencies, may be required to report to a designated location.
• Performs other duties as assigned.
