Chapters Health System is seeking a Per Diem RN in Florida to assess patient and family needs, implement the nursing process, and coordinate care with the Interdisciplinary Group. The role focuses on palliative and supportive care within hospice settings.

Qualified candidates hold an active RN license in Florida, BLS certification, and EMR/EHR experience; a valid driver’s license and reliable transportation are required. Flexible schedule and strong communication skills are essential.

Formación

• Current RN license in the state of employment.
• Active BLS for healthcare professionals (AHA or Red Cross).
• Minimum 1 year nursing experience in hospice or hospital settings preferred.
• Experience with EMR/EHR systems.

Responsabilidades

• Provide direct patient and family care leveraging the interdisciplinary plan.
• Coordinate Plan of Care with IDG to address evolving patient needs.
• Educate patients and families on disease, care options, and safety practices.
• Communicate changes in patient condition to the IDG and physicians.
