Job summary

Gentiva is seeking a Patient Care Manager to lead clinical operations in our hospice team. You will supervise patient care delivery, schedule visits, triage referrals, and guide IDG meetings to ensure compliant, high-quality services.

The role requires RN licensure, hospice experience, and strong leadership and documentation skills. Full-time, on-site, with comprehensive benefits and salary range of $100,000–$115,000.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years direct patient care, incl. 2 years hospice/home-based care.
• Demonstrated hospice principles, standards, and regulatory compliance.
• Knowledge and compassion for terminally ill patients and families.
• Ability to lead a diverse care team and adapt to varied backgrounds.
• Strong interpersonal, organizational, and leadership skills.
• Competency in clinical documentation and technology systems.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritize based on clinical judgment.
• Triage incoming referrals and medical concerns promptly and compassionately.
• Lead or facilitate IDG meetings and document minutes.
• Mentor and train associates, supporting development and compliance.
• Ensure care delivery meets regulatory standards and company policies.
• Support performance improvement initiatives and operational efficiencies.
