Job summary

You will report to the Executive Director or Administrator, overseeing care delivery and regulatory adherence across the location, while supporting performance improvement and staff development.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program with relevant licensure.
• Minimum 3 years of direct patient care, incl. hospice/home-based care.
• Demonstrated knowledge of Medicare/Medicaid, JCAHO, ACHC standards.
• Ability to lead a diverse care team and manage clinical documentation.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritize based on clinical judgment.
• Triage referrals and medical concerns promptly with compassion.
• Lead or facilitate IDG meetings and document minutes.
• Mentor and train associates to support performance and compliance.
• Ensure care delivery meets regulatory standards and company policies.
• Support performance improvement initiatives and operational efficiency.
