Job summary

Gentiva in Pittsburgh is seeking a Patient Care Manager, RN to lead hospice clinical operations. You will oversee patient care delivery, coordinate visits, and ensure regulatory compliance while supporting a compassionate care team.

You will mentor staff, lead IDG meetings, and drive quality improvement, with emphasis on hospice principles and family-centered care. This full-time role requires a state RN license and at least three years of direct patient care, including hospice/home health.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years direct patient care, incl. hospice/home health.
• Demonstrated knowledge of hospice principles, Medicare/Medicaid, JCAHO/ACHC.
• Ability to lead a diverse care team with strong communication.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and clinical triage with sound judgment.
• Triage referrals and coordinate timely care coordination.
• Lead IDG meetings and document minutes.
• Mentor staff and ensure regulatory compliance across care delivery.
• Support performance improvement and quality initiatives at your location.
