Job summary

Gentiva Hospice is seeking a dedicated Patient Care Manager to oversee hospice patient care activities and ensure high-quality, compliant services. You will lead clinical operations, coordinate care, and mentor staff to uphold standards.

The role reports to the Executive Director or Administrator and emphasizes IDG leadership, regulatory adherence, and continuous improvement in patient outcomes across the location.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years of direct patient care experience, including 2 years in hospice or home-based care.
• Demonstrated knowledge of hospice principles, regulatory compliance (Medicare/Medicaid/JCAHO/ACHC).
• Compassionate approach to terminally ill patients and families.
• Ability to lead a diverse care team in varied emotional/clinical settings.
• Strong interpersonal, organizational, and leadership skills.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritise visits based on clinical judgment.
• Triage referrals and medical concerns promptly and compassionately.
• Lead or facilitate Interdisciplinary Group (IDG) meetings and document minutes.
• Mentor and support associates in development, performance, and compliance.
• Ensure care delivery meets federal, state, local regulations, and policies.
• Support performance improvement initiatives and operational efficiencies.
