Job summary

Gentiva Hospice in California seeks a Patient Care Manager (RN) to lead clinical operations and ensure high-quality, compliant hospice services. Reporting to the Executive Director, you will oversee patient care, coordinate visits, mentor staff, and drive regulatory compliance at our San Diego location.

The ideal candidate holds a current California RN license, 3+ years of direct patient care (2+ in hospice/home health), and strong leadership, communication, and documentation skills.

Qualifications

• RN license current in California
• Graduate of an accredited nursing program
• Minimum 3 years direct patient care, incl 2 years hospice/home health
• Knowledge of hospice principles and regulatory compliance
• Strong leadership, interpersonal, organizational skills
• Competency in clinical documentation and technology systems

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care
• Manage visit scheduling and prioritize visits based on clinical judgment
• Triage referrals and medical concerns promptly and compassionately
• Lead IDG meetings, present patient updates, coordinate care plans, document minutes
• Mentor, train, and provide clinical support to associates
• Ensure care delivery meets regulatory standards and company policies
• Support performance improvement and operational efficiency at the location
