Job summary

Heartland Hospice, part of Gentiva, seeks a Patient Care Manager to lead clinical operations and coordinate high-quality hospice care. You will supervise care delivery, scheduling, and regulatory compliance while mentoring staff across the location.

The role requires an RN license with hospice/home health experience and strong leadership, communication, and documentation skills to support compassionate, compliant care in patients\' homes and facilities.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years direct patient care, including 2 years in hospice or home health.
• Knowledge of hospice principles, regulatory compliance (Medicare, Medicaid, JCAHO, ACHC).
• Compassion for terminally ill patients and families.
• Ability to lead a diverse care team and adapt to varied patient needs.
• Strong interpersonal, organizational, and leadership skills.
• Competency in clinical documentation and technology systems.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritize visits based on clinical judgment.
• Triage referrals and medical concerns promptly and compassionately.
• Lead IDG meetings, present updates, review charts, and document minutes.
• Mentor and train associates, supporting development and compliance.
• Ensure care delivery meets regulatory standards and company policies.
• Support performance improvement and operational efficiencies at the location.
