Job summary

Gentiva Hospice in Seattle seeks a Patient Care Manager to oversee hospice services, lead clinical operations, and ensure compliant, high-quality care across all visits. Reporting to the Executive Director, you will mentor staff, coordinate IDG meetings, and drive performance improvement while maintaining regulatory standards and patient- and family-centered care.

Qualifications

• Current RN license in the state of employment must be maintained.
• Graduate of an accredited nursing program with licensure.
• Minimum 3 years of direct patient care, including 2 years in hospice/home health.
• Knowledge of hospice principles, regulatory compliance (Medicare/Medicaid/JCAHO/ACHC).
• Compassion for terminally ill patients and families; strong leadership.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritize based on clinical judgment.
• Triage referrals and medical concerns promptly and compassionately.
• Lead IDG meetings, present updates, coordinate care plans and document minutes.
• Mentor and support associates; ensure compliance with policies and standards.
• Ensure care delivery meets regulatory and company requirements; drive improvements.
