Gentiva Hospice is seeking a Patient Care Manager to lead hospice clinical operations. You will oversee patient care, coordinate visits, triage referrals, and lead IDG meetings, ensuring regulatory compliance and high-quality service.

The role requires an active RN license in the employment state, 3+ years of direct patient care with hospice/home health exposure, and strong leadership skills to mentor staff and manage care teams.

Qualifications

• Must hold an active RN license in the state of employment.
• Graduate of an accredited nursing program with active licensure.
• Minimum 3 years of direct patient care experience, including 2 years in hospice or home-based care.
• Knowledge of hospice principles and regulatory compliance (Medicare/Medicaid/JCAHO/ACHC).

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling, including reassigned, missed, declined, or rescheduled visits using clinical judgment.
• Triage incoming referrals and medical concerns promptly and compassionately.
• Lead Interdisciplinary Group (IDG) meetings and coordinate care plans, documenting minutes.
• Mentor and provide clinical support to associates, supporting development and compliance.
• Ensure care delivery meets all regulatory standards and company policies.
• Support performance improvement initiatives and departmental operations.
