Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years of direct patient care experience, including 2 years in hospice or home-based care.

Responsibilities

• Oversee and direct patient care services to ensure the delivery of high-quality hospice care.
• Manage visit scheduling and prioritization based on clinical judgment.
• Triage incoming referrals and clinical concerns promptly and compassionately.
• Lead IDG meetings and document minutes with care plans.
• Mentor and support associates to ensure regulatory compliance and performance.
• Ensure care delivery meets federal, state, and local standards and policies.
• Support quality improvement and operational efficiency at the location.
