Job summary

Gentiva in Seattle seeks a compassionate Hospice RN / RN Case Manager to guide patients and families through the hospice journey, delivering personalized in-home nursing care and coordinating with an interdisciplinary team. You will perform assessments, develop plans of care, educate caregivers, and maintain clinical documentation while upholding quality and regulatory standards.

Qualifications

• Current license to practice nursing in the state of employment.
• Minimum of one year RN experience in acute care, oncology, long‑term care, hospice, or home health.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and ongoing RN assessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized care plans and visit schedules.
• Coordinate care with IDG and physicians.
• Educate patients, families, and caregivers on disease, end-of-life care, and medications.
• Document clinical records and ensure compliance.
