Gentiva is seeking a Patient Care Manager, RN Clinical Manager, to lead hospice operations in New Jersey. You will oversee patient care delivery, coordinate visits, and mentor staff while ensuring regulatory compliance and high-quality service.

The role calls for RN licensure, hospice experience, and strong leadership in a dynamic clinical setting. This is a full-time position with comprehensive benefits and a focus on compassionate, compliant patient care.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years direct patient care experience, including 2 years in a hospice or home-based care setting.
• Demonstrated knowledge of hospice principles, industry standards, and regulatory compliance (Medicare, Medicaid, JCAHO, ACHC).
• Knowledge and compassion for terminally ill patients and their families.
• Ability to lead a diverse care team while adapting to emotional and clinical needs.
• Strong interpersonal, organizational, and leadership skills.
• Proficiency in clinical documentation and technology systems.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling, including reassigned, missed, declined, or rescheduled visits based on clinical judgment and priority.
• Triage incoming phone referrals and medical concerns promptly and compassionately.
• Lead or facilitate Interdisciplinary Group (IDG) meetings and coordinate care plans.
• Mentor, trainer, and clinical support to associates, supporting development, performance, and compliance.
• Ensure all care delivery meets federal, state, and local regulatory standards and company policies.
• Support performance improvement initiatives, quality assessments, and operational efficiencies at the location.
