Job summary

Advance Hospice Care is seeking a Patient Care Manager to oversee clinical operations, coordinate care, and develop staff to uphold high-quality, compliant hospice services. You will report to the Executive Director or Administrator and lead care delivery with a focus on regulatory compliance and patient-centered outcomes.

You will direct patient care, triage referrals, and facilitate IDG meetings while mentoring associates and ensuring adherence to federal and state requirements.

Qualifications

• Licensed RN in the state of employment.
• Graduate of an accredited nursing program.
• Minimum 3 years direct patient care, including 2 years hospice/home health.
• Knowledge of hospice principles and regulatory compliance (Medicare/Medicaid).
• Ability to lead a diverse care team with strong leadership.

Responsibilities

• Oversee and direct patient care to ensure high-quality hospice services.
• Manage visit scheduling and clinical prioritization based on urgency.
• Triage referrals and medical concerns promptly and compassionately.
• Lead Interdisciplinary Group meetings and document minutes.
• Mentor staff and provide clinical support for development and compliance.
• Ensure care delivery meets regulatory standards and company policies.
• Support performance improvement and regulatory compliance initiatives.
