Job summary

Gentiva is seeking a Senior Hospice RN Patient Care Manager to lead patient care operations, supervise clinical staff, and ensure compliance with Medicare/Medicaid and ACHC standards. Reporting to the Executive Director, you will oversee referrals, scheduling, and clinical documentation.

The role requires an active Georgia RN license, an Associate degree (BSN preferred) and 3+ years of direct patient care, with EMR proficiency and strong communication.

Qualifications

• Current RN license in Georgia.
• Graduate of an accredited nursing program (Associate degree accepted with relevant experience).
• Minimum 3 years direct patient care experience.
• Knowledge of hospice regulatory compliance and managed care.
• Strong communication, collaboration, and organizational skills.
• Proficiency with EMR, Microsoft Office, and healthcare software.
• CPR certification and valid driver's license.

Responsibilities

• Lead hospice patient care operations to ensure quality and compliance.
• Supervise, teach, evaluate, and develop nursing and clinical staff.
• Manage referrals, DME pickups, Medicare eligibility, insurance verification, and patient benefits.
• Coordinate IDG meetings to review care plans and update documentation.
• Oversee scheduling, visit assignments, and timely patient visits.
• Serve as liaison between clinical teams, leadership, and external providers.
• Ensure adherence to Medicare, Medicaid, JCAHO, ACHC regulations.
