Job summary

Compassus is seeking a Nurse Practitioner - PPV to model our core values and promote compassionate, high-quality palliative and hospice care. You will perform assessments, provide education, and manage symptoms for patients and families across assigned locations, collaborating with the Medical Director and IDT to ensure optimal care.

Qualifications

• Master’s degree in Nursing is required.
• Active Nurse Practitioner license is required.
• Board Certification in Hospice and Palliative Care is preferred.
• Active NPI number is required.

Responsibilities

• Provide assessments, disease and symptom education, and plan of care for palliative/hospice patients.
• Collaborate with Medical Director, Attending Physician, and IDT to coordinate care.
• Maintain patient records and ensure compliance with practice standards.
• Prescribe and manage medications within scope of practice.
• Educate patients and families about disease processes and care planning.
