Job summary

Providence at Home with Compassus is seeking a Nurse Practitioner to provide palliative and hospice care across patient care settings. You will perform routine and emergency assessments, educate families, and collaborate with the PCP and interdisciplinary team to maintain comfort and quality of life. The role may involve on-call coverage and after-hours admissions.

The NP will work with the Medical Director and IDT to ensure adherence to standards and best practices in end-of-life care.

Responsibilities

• Performs routine and emergency assessments, including recertification visits for patients entering the 3rd benefit period or beyond.
• Communicates with Medical Director, Attending Physician, DCS and IDT regarding findings and symptom management.
• Provides physical and emotional care to support the patient and family in crisis.
• Acts as attending practitioner for patients at admission, coordinating with patient, family, IDT and physician on symptom management and end-of-life care.
• Rotates into on-call schedule with physician backup for admissions.
• Performs routine and emergency outpatient assessments related to palliative care visits.
• Conducts bedside palliative care evaluations in the patient’s residence.
