Job summary

Compassus is seeking a Nurse Practitioner specialized in palliative and hospice care. The NP will perform routine and emergency assessments, coordinate with the IDT, and manage symptom control and patient comfort in collaboration with physicians.

Responsibilities include order interpretation, documentation, and patient/family education. The role requires a Master's in Nursing and an active NP license; board certification in Hospice and Palliative Care is preferred.

Qualifications

• Master’s degree in Nursing is required.
• Licensure as a Nurse Practitioner with active license in state of employment is required.
• Board Certification in Hospice and Palliative Care is preferred but not required.
• Active NPI number and DEA certification if state allows prescription of controlled substances are required/maintained.

Responsibilities

• Performs routine and emergency assessments for hospice and palliative care patients.
• Collaborates with Medical Director, Attending Physician, DCS and IDT to coordinate patient care.
• Orders, performs, and interprets laboratory and radiology tests within scope.
• Prescribes medications and orders treatments and equipment.
• Maintains accurate patient records and documentation for reimbursement and care planning.
• Ensures compliance with applicable laws, regulations, and Collaborative Agreements.
• Supports patients and families in goals of care and advanced care planning.
• Supervises trainees and participates in education and supervision activities.
