Compassus seeks a Nurse Practitioner to provide palliative and hospice care within patients' homes and affiliated settings. The NP collaborates with the medical director, IDT, and PCP to manage symptoms, educate families, and plan end-of-life care.

Qualifications

• Master's degree required for Nursing practice.
• Active CNS or Nurse Practitioner license required.
• Board certification in Hospice and Palliative Care preferred but not required.

Responsabilités

• Performs routine and emergency assessments and visits.
• Communicates with Medical Director and IDT regarding findings and symptom management.
• Provides physical and emotional care to patient and family during crises.
• Acts as attending practitioner for admitted patients and collaborates with the care team.
• Completes required paperwork within 24 hours of visits.
