Job summary

Compassus in Texas seeks a Nurse Practitioner - PPV to deliver palliative and hospice care aligned with Compassus values. You will assess, educate, and manage symptoms for patients and families while collaborating with PCPs and the interdisciplinary team to ensure high-quality, compliant care.

The role requires a state NP license and flexibility to meet agency needs, including after-hours visits. You will play a key part in care coordination and education across patient settings.

Qualifications

• Master’s degree in Nursing required.
• Nurse Practitioner license in state of employment.
• Experience in palliative or hospice care preferred.

Responsibilities

• Provide assessments, education, and symptom management for palliative care patients.
• Collaborate with PCP and IDT to coordinate care.
• Flex schedule to meet agency needs and after-hours visits as required.
• Complete documentation and ensure compliance with practice standards.
• Assist with hospice recertification and emergency assessments when needed.
