Job summary

VIA Health Partners is seeking a Nurse Practitioner to deliver comprehensive medical care to hospice and palliative patients within our community‑based, not-for-profit program. You will assess, diagnose, and treat complex symptoms, while guiding families through prognosis and care options.

Qualifications

• Minimum 1 year related experience. Experience in direct care of patients with life limiting illnesses required.
• Background in hospice or palliative care preferred.
• Ability to work within a community-based, nonprofit healthcare setting.

Responsibilities

• Promotes high quality end-of-life care and use of advanced directives.
• Diagnoses, treats, and manages pain and other symptoms.
• Guides patients and families on prognosis, goals of care, tests, and therapies.
• Prescribes therapies and coordinates with Medical Director as needed.
• Acts as an education resource within the organization and community.
