Job summary

Elite Hospice, part of the LHC Group and within the Optum family, is seeking a Hospice Nurse Practitioner to deliver palliative and supportive care. You will consult with the Hospice Medical Director, coordinate plans of care with families and physicians, and lead patient/family education.

The role emphasizes clinical leadership, thorough documentation, and collaboration with an interdisciplinary team to improve patient quality of life in hospice settings.

Qualifications

• Active unrestricted license in the state of residence.
• Current certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practice.
• Current national certification (ANCC, NBCHPN, or AANP).
• 5+ years nursing experience.
• Valid driver's license and access to a vehicle or reliable transit.

Responsibilities

• Perform face-to-face hospice visits within prescribed time frames and report findings to the Hospice Medical Director.
• Collaborate with the Hospice Medical Director and referring physicians; coordinate services.
• Develop and document the plan of care; monitor patient/family response to hospice care.
