Job summary

Bon Secours Home Care and Hospice by Compassus is seeking a Nurse Practitioner to provide hospice or palliative care within home-based settings. You will assess, educate, and manage symptoms for patients and families, collaborating with the Medical Director and IDT to deliver comprehensive care.

The role requires a Master’s in Nursing and an active NP license, with board certification in Hospice/Palliative Care preferred. Travel to patient locations and flexible scheduling are part of the duties.

Qualifications

• Master’s degree in Nursing is required.
• Active Nurse Practitioner license in the employment state required.
• Board certification in Hospice & Palliative Care preferred.
• Ability to travel to patient locations as needed.

Responsibilities

• Perform routine and emergency assessments for hospice or palliative care patients.
• Coordinate care with Medical Director, attending physicians, DCS and IDT.
• Provide patient and family education, symptom management, and support at home.
• Maintain accurate patient records and ensure CMS/compliance standards.
