Compassus seeks a Nurse Practitioner to support hospice and palliative care patients across locations in the Des Moines area. You will perform routine and emergency assessments, provide symptom management and education, and collaborate with the Medical Director, attending physicians, and the interdisciplinary team to ensure high-quality patient care.

The role requires an NP license, NP certification in applicable areas, and availability for flexible scheduling, including after-hours as needed.

Qualifikationen

• Master’s degree in Nursing required.
• Active and unencumbered NP license in state of employment required.
• Certified as NP in Adult, Family, Acute, or Geriatric required.
• Active NPI number.
• DEA certification may be required for controlled substances.
• Board Certification in Hospice/Palliative Care preferred but not required.
• All states except Iowa require current collaborative physician agreement.

• Performs routine and emergency assessments for hospice or palliative care patients.
• Collaborates with Medical Director, Attending Physician, DCS and IDT.
• Provides patient and family education and support.
• Maintains accurate patient records and documentation.
• Adheres to confidentiality and regulatory requirements.
