Bethany Hospice & Palliative Care is seeking a full-time Community Relations Liaison to educate the community about hospice and palliative care, and to support timely referrals. The role emphasizes relationship-building with physicians, hospitals, and senior living communities while upholding patient-centered care.

The ideal candidate is self-directed, skilled at navigating complex healthcare environments, and committed to integrity, compassion, and professionalism in representing our mission.

Competenze

• Bachelor’s degree in healthcare, Nursing, Business Administration or related field (Preferred).
• One or more years of health care or social service experience (Preferred).
• Previous sales, referral or customer service experience (Required).
• Ability to market collaboratively with customers, referral sources, and the community.

Mansioni

• Build and maintain strong relationships with physicians, hospitals, senior living communities, and referral sources.
• Educate the community about hospice and palliative care services, eligibility, and benefits.
• Promote timely hospice referrals aligned with patient goals of care and eligibility requirements.
• Complete hospice consents and collaborate with clinical team to ensure admission continuity.
